Job summary

Mary Bird Perkins Cancer Center in Louisiana seeks an experienced Oncology Social Work Navigator to advocate for patients and families, guiding them through the cancer care continuum. The role emphasizes patient-centered care, collaboration with clinicians, and support services coordination.

The position requires a Master’s in Social Work and an LMSW with LCSW and OSW-C credentials preferred. This role focuses on reducing barriers and improving survivorship across the cancer service line.

Qualifications

• Master's degree in Social Work.
• Experience in oncology preferred or related field through previous job, internship, or volunteer program.
• LMSW required, LCSW preferred; OSW-C encouraged.

Responsibilities

• Develops strong relationships with physicians, their office staff, and healthcare providers to ensure effective communication in patient care.
• Coordinates all aspects of patient care and development related to the cancer service line.
• Educates the patient and caregiver on disease-specific guidelines and treatment options to enable patient-led treatment decisions.
• Coaches and assists patients to remove barriers related to insurance coverage, transportation, finances, language, etc.
• Implements risk assessments and monitors healthcare pathways with multidisciplinary teams.
• Participates in performance improvement activities to enhance patient safety and care quality.
• Assists with selection of student interns and provides supervision as assigned.
