Job summary

Intermountain Health seeks an RN Navigator—Oncology to guide patients through their cancer care journey. You will advocate for patients, coordinate education, discharge planning, and follow-up services, and partner with physicians and care teams to ensure seamless, high-quality cancer care.

You will work on-site in Grand Junction, CO, with a 40-hour week and a pay range aligned to experience. The role requires RN licensure, BLS, and at least three years of RN experience.

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Current BLS certification (or obtained within 60 days of hire).
• Minimum of 3 years RN experience.
• BSN Oncology or related specialty certification preferred.

Responsibilities

• Guide patients and families through the oncology care process with clear coordination.
• Assess needs and create individualized care plans supporting goals.
• Coordinate education, discharge planning, post-acute care, and follow-up services.
