Job summary

AdventHealth in Orlando, FL seeks an RN to actively participate in discharge planning, coordinating post-acute care, and educating patients and families. You will document plans, communicate with payors, and support timely transitions with a focus on holistic patient care.

The role requires an Associate's in Nursing (Bachelor's preferred) and at least 2+ years of hospital nursing experience, with CCM/ACM preferred. No remote work; full-time days.

Qualifications

• RN with required nursing degree and active license.
• Experience with discharge planning and care coordination preferred.
• Ability to assess clinical, social, and financial factors in discharge.

Responsibilities

• Participate in rounds to review patient status and discharge plans.
• Educate patients and families on health care decisions and resources.
• Document discharge planning evaluations and plans.
• Coordinate with post-acute care and payors for authorizations.
• Communicate with multidisciplinary team to ensure timely care progression.
• Perform continuous discharge planning assessments in inpatient/ED settings.
• Leverage EMR systems for continuity of care.
