Job summary

AdventHealth in Orlando, FL seeks an experienced RN for discharge planning and utilization management. You will lead multidisciplinary rounds, coordinate post-acute care, and ensure timely transitions with CMS-compliant discharge planning.

The role requires strong leadership, data analysis, and communication with diverse families and payors. The position is full-time, days, with 40 hours weekly and weekend rotation, located on our Lake Nona campus expansion project.

Qualifications

• RN with at least an Associate degree in Nursing
• Bachelor's degree in Nursing preferred but not required
• Proven leadership and clinical coordination abilities
• Experience in discharge planning and post-acute care coordination
• Strong knowledge of CMS Conditions of Participation for Discharge Planning

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status and discharge plans.
• educates patients and families on the impact of illness and community resources.
• Assess readmitted patients for reasons and quality of care needs.
• Organizes and facilitates patient and family care conferences with the team.
• Documents discharge planning evaluations and barriers to progression of care.
• Develops discharge plans with contingency plans for post-acute care.
• Communicates with post-acute services to ensure continuity of care.
