Qualifications

• High school diploma or equivalent.
• Three to five years experience in a pharmacy with IV skills (Level 2).
• Five to seven years experience in a pharmacy with IV skills (Level Sr).

Responsibilities

• Assist with preparation of medication orders by compounding or admixing medications under supervision.
• Check orders for completeness and verify patient information and physician authorization.
• Admix drugs as ordered by a physician in an accurate and timely manner.
• Calculate dosage volumes and convert units as needed.
• Maintain temperature logs and protocol/patient assistance drug records.
• Manage inventory and document lot numbers and expiration dates.
• Ensure cleaning and maintenance of the admixture area and pharmacy per state rules.
