Job summary

Trinity Health Des Moines seeks a Clinical Pharmacist to develop and provide pharmaceutical care with multidisciplinary teams. You will promote medication use policies and assist with department and committee policies, while educating patients, students, residents, pharmacists, and staff.

The role emphasizes leadership to advance clinical practice, maintain professionalism and confidentiality, and support the Pharmacy and Therapeutics Committee.

Qualifications

• Pharmacist degree from ACPE-accredited college.
• Completion of ASHP-accredited PGY-1 or equivalent experience preferred.
• PGY-2 residency, Fellowship, or 3+ years in specialty practice recommended.
• Iowa license or eligibility required within 3 months of hire.
• BLS/ACLS certification due within 3 months of hire.
• Board certification in specialty within 1 year of hire.

Responsibilities

• Staff in designated patient care areas performing clinical pharmacist duties (40–80%).
• Support Pharmacy and Therapeutics Committee, formulary planning and maintenance.
• Lead or participate in Medication Use Evaluation (MUE) and quality improvement efforts.

Education

Pharmacy degree (BS or Pharm.D.)

PGY-1 Pharmacy Practice Residency or equivalent

PGY-2 Specialty residency or Fellowship or 3+ years of practice

Licensed as Pharmacist in Iowa or eligible

BLS certification within 3 months

ACLS certification within 3 months

BPS Board Certification in specialty area within 1 year

Mandatory Reporter abuse training completion
