Job summary

You will participate in interdisciplinary rounds, provide medication counseling, manage medication histories and discharge planning, and support drug information and dosing activities as part of a collaborative health-care team.

Responsibilities

• Provide patient education on medications and device use, while counseling and answering drug information needs.
• Participate in interdisciplinary rounds and collaborate with the treatment team to optimize outcomes.
• Perform medication history interviews and discharge counseling; update patient medication lists at discharge.
• Monitor therapies through chart reviews, lab value interpretation, and adherence assessment.
• Teach and precept trainees during IPPE rotations.
• Process cash/credit transactions and assist with vaccination preparation and administration as applicable.
