Job summary

Qualifications

Responsibilities

• Provide patients with information on prices, refills, policies and insurance.
• Participate in interdisciplinary rounds with the treatment team.
• Perform medication history interviews, dosing checks, counseling at discharge and education assessment.
• Review medication orders for dose, duration, frequency, indication and interactions.
• Create and update patient medication lists at discharge or clinic visit.
• Administer vaccinations when applicable and prepare extemporaneous compounds as needed.
