Job summary

OhioHealth is seeking a dedicated clinical pharmacist for the Bone Marrow Transplant-Hematology program. The role combines direct patient care with policy and formulary duties, including quality improvement and guideline development.

The position rotates through inpatient and outpatient services, with a focus on evidence-based, value-driven oncology therapy. Essential qualifications include a PharmD or BS, Ohio license, BCOP or eligibility within 24 months, and pharmacy residency or hospital

Qualifications

• Pharmacist licensed in Ohio (OPL).
• BS or PharmD degree from an accredited program.
• Completion of a Pharmacy Practice residency or equivalent hospital experience.
• Board Certification in Oncology (BCOP) or eligible within 24 months.
• BCOP certification must be maintained throughout employment.
• Pharmacokinetic drug dosing experience.

Responsibilities

• Review oncology medication therapy in collaboration with the care team.
• Recommend modifications to therapy based on patient data and records.
• Adjust chemotherapy plans in EHR for age and performance status.
• Communicate pharmacist interventions and provide alternatives to treatment plans.
• Educate patients and staff; mentor Doctor of Pharmacy Students.
