Qualifications

Responsibilities

• Perform chemotherapy regimen verification for ordering accuracy, dosing, and administration parameters.
• Assess and manage toxicities and recommend monitoring.
• Provide evidence-based recommendations for oncology agents and regimens.
• Conduct comprehensive medication management including interactions and deprescribing as appropriate.
• Oversee aseptic compounding of hazardous and non-hazardous sterile preparations per USP 797/800.
• Ensure proper donning and CSTDs and beyond-use dating; maintain bar-code verification.
• Lead or support medication-use evaluations, incident reviews, and quality actions.
• Develop and update chemotherapy protocols, order sets, and pathways.
• Practice under collaborative practice agreements to initiate/adjust therapies.
• Support clinical trials, verify protocol regimens, and ensure protocol compliance.
• Educate and mentor staff; contribute to policies and training.
• Maintain EHR chemotherapy plans and decision support; review alerts and dosing calculators.

Oncology Pharmacist

The Oncology Pharmacist provides specialized pharmaceutical care to patients receiving chemotherapy, immunotherapy, targeted agents, and other complex cancer treatments. This role is responsible for verifying antineoplastic regimens, managing supportive care, preventing and resolving medication-related problems, and ensuring safe preparation and handling of hazardous drugs. Working collaboratively with oncologists, nurses, and the interdisciplinary care team, the Oncology Pharmacist supports optimal, evidence based therapy, enhances patient outcomes, and promotes medication safety throughout the oncology service line.

1) Clinical Care & Therapeutic Management

• Perform prospective chemotherapy regimen verification for ordering accuracy, indication, dosing (BSA/weight/renal/hepatic), sequencing, diluents, stability, and administration parameters.
• Assess and manage toxicities (e.g., myelosuppression, mucositis, neuropathy, cardiotoxicity), implement dose adjustments, and recommend monitoring.
• Provide evidence-based recommendations for antineoplastic agents, targeted therapies, immunotherapies, cellular therapies, and clinical trial regimens.
• Conduct comprehensive medication management (CMM): reconcile medications, evaluate drug–drug and drug–gene interactions, and deprescribe when appropriate.

2) Sterile Compounding & Safe Handling

• Oversee and/or perform aseptic compounding of hazardous and non‑hazardous sterile preparations per USP <797> and USP <800>.
• Ensure proper donning, engineering controls, closed‑system transfer devices (CSTDs), and beyond‑use dating.
• Maintain chain-of-custody, labeling, and bar‑code verification for chemo admixtures.

3) Medication Safety, Quality, & Compliance

• Lead or support medication-use evaluations (MUEs), incident reviews, and corrective actions related to oncology medications.
• Develop, implement, and update chemotherapy protocols, order sets, and pathways to reflect current best evidence.
• Ensure regulatory readiness (e.g., TJC/Det Norske Veritas, USP chapters, state board) and maintain documentation/audits.
• Track and report clinical interventions, outcomes, cost avoidance, and safety metrics.

4) Collaborative Practice & Protocols

• Practice under collaborative practice agreements (CPAs) to initiate/adjust therapies, order labs, and manage supportive care within scope.
• Coordinate care transitions with infusion center, inpatient units, specialty pharmacy, home infusion, and community providers.

5) Research, Trials, & Innovation

• Support clinical trials : verify protocol regimens, manage investigational products (IP), and ensure protocol compliance and documentation.
• Evaluate new drugs and indications for formulary review and safe implementation.

6) Education, Training, & Leadership

• Precept and mentor oncology pharmacy technicians.
• Provide in-services to nursing, providers, and ancillary staff on oncology medications and safety.
• Contribute to departmental policies , strategic initiatives, and interprofessional workgroups.

7) Informatics & Technology

• Build, validate, and maintain EHR chemotherapy plans, order sets, and clinical decision support (alerts, dosing calculators).
• Utilize BCMA and smart pump libraries to ensure accurate dispensing and administration; elevate issues promptly.

8) Documentation & Communication

• Communicate clearly with prescribers, nurses, and the infusion team to resolve order issues and ensure continuity of care.
• Report adverse drug events (ADEs), near misses, and pharmacovigilance data per internal/external requirements.

9) Operational Excellence

• Prioritize and triage STAT, time‑sensitive chemotherapy and supportive care orders.
• Ensure adequate inventory, storage, and waste management of hazardous drugs; coordinate drug shortage mitigation plans.
• Assist with billing accuracy (NDC mapping, waste documentation, HCPCS coding support) to minimize denials.
• Additional Duties to include Order verification at Central Pharmacy, IV room Pharmacist duties at Central Pharmacy, or other additional duties as assigned.

Bachelor’s degree or Pharm D from an accredited College of Pharmacy.
