Job summary

Tallahassee Memorial HealthCare seeks a pharmacist to provide safe, effective medication therapy for hematology/oncology patients, partner with multidisciplinary teams on rounds, and support outpatient care at the Tallahassee campus.

The role requires a Pharm.D., Florida licensure or eligibility, and PGY1/PGY2 residency, with 1 year of relevant experience and 2–4 years preferred. BLS certification and BCOP/BCPS credentials are desirable.

Qualifications

  • Pharm.D. degree from ACPE-accredited school.
  • Post-graduate residency training preferred (PGY1/PGY2).
  • Active pharmacist licensure in Florida or eligibility required.

Responsibilities

  • Provide safe, effective medication therapy to hematology/oncology patients.
  • Participate in rounds and outpatient coverage; support on-call/weekend coverage.
  • Lead formulary management and medication therapy optimization initiatives.
  • Engage in QA activities to evaluate pharmacotherapy impact.

  • Assesses and evaluates medical and drug literature critically;develops/leads formulary management, medication therapyrecommendations, and optimization initiatives.

  • Participates in qualityassurance (QA) activities (e.g., drug utilization evaluations, qualityimprovement (QI) initiatives, and evaluating clinical pharmacists’ impact ondrug therapy measurements).

  • Establishes and/or maintains a viable practicesuitable for educating and training.

  • Performs Pharmacist duties (asrequired).

  • Performs other duties.

  • None. Oversees the work of, but does not directly supervise, the following:Pharmacy Technician I/Non-Cert (JC: 072205), Pharmacy Technician II/Cert(JC: 072305), Pharmacy Technician III/Cert (JC: 072405), and PharmacyTechnician IV/Cert (JC: 072705).

WHAT YOU’LL NEED TO APPLY

Required Education:

  • Doctor of Pharmacy (Pharm.D.) degree from an Accreditation Council for Pharmacy Education (ACPE) accredited school of pharmacy

Preferred Education:

  • Post-Graduate Year 1 (PGY1) Pharmacy Residency completion. Post-Graduate Year 2 (PGY2) Pharmacy Residency (in specialization area) completion.

Required Experience:

  • One (1) year of relevant registered pharmacist work experience

Preferred Experience:

  • Two (2) to four (4) years of clinical pharmacy experience.

Required Certification/License/Registry:

  • Licensed [or eligible for licensure (i.e., meets all statutory requirements for licensure application)] as a pharmacist in accordance with Florida Statutes (F.S.), Title XXXII (Regulations of Professions and Occupations), Chapter 465 (Pharmacy), Sections 007 through 013. Basic Life Support (BLS) certification from an American Heart Association (AHA) or American Red Cross (ARC) recognized provider. Board of Pharmacy Specialties (BPS) Board Pharmacy Specialty Certification (BCOP) and/or BPS Board Certified Pharmacotherapy Specialist (BCPS) credential(s).
