Job summary

• Clinically triage new patient referrals to ensure timely access to care.
• Serve as primary contact for patients, providing advocacy and education.
• Coordinate multidisciplinary care and expedite patient workups and treatment plans.
• Educate patients about care plans, medications, and processes; act as liaison between patients and providers.
• Collaborate with hospital staff and community resources to improve access and outcomes.
• Participate in outreach and education activities within the community.
