Job summary

Palo Verde Community College District seeks a Patient Navigator-Registered Nurse (RN) in the Cancer Center to provide age-appropriate oncology care for adults. The nurse will case manage from referral through treatment in collaboration with physicians and the multidisciplinary team.

Qualifications

• Bachelor of Science in Nursing required (in progress acceptable).
• Minimum one year of nursing experience required.
• Minimum one year of oncology nursing preferred.
• One year case management or transitional care management experience preferred.

Responsibilities

• Coordinate the continuum of care from referral through treatment with the physician and multidisciplinary team.
• Proactively manages daily clinic activities to ensure efficient and timely patient visits.
• Implements research protocols in collaboration with the research team.
