Qualifications

Responsibilities

• Plan, implement, coordinate and promote continuity of care.
• Assess health status by interviewing patients and reviewing history.
• Administer therapies under physician supervision; manage medications.
• Maintain patient confidentiality and accurate documentation.
• Provide psychosocial support to patients and families; coordinate care.
