Job summary

Stanford Health Care in California seeks an experienced Oncology Nurse Navigator to ensure guideline-based care across NCCN pathways and available clinical trials. You will educate patients on disease options and treatments, and coordinate care with the multidisciplinary team.

You will collaborate with social work, nutrition, and IT systems to support survivorship and palliative care as needed, guiding patients and families through complex care journeys.

Qualifications

• Registered Nurse with California license
• Minimum 5 years of nursing experience
• Minimum 2–3 years oncology and nurse navigation experience preferred

Responsibilities

• Ensure care follows NCCN and Stanford pathways, including trials.
• Assess patient education needs and provide disease-specific information.
• Address barriers to care and coordinate services for at-risk populations.
• Collaborate with social work and nutrition to reduce barriers.
• Provide one-on-one nurse navigation for patient, family and caregiver needs.
• Facilitate referrals for second opinions and local treatment options.
• Deliver culturally appropriate education materials and services.
• Screen for distress and refer to support services.
• Coordinate care from diagnosis to survivorship per guidelines.
• Support accreditation activities and quality standards.
• Represent Stanford Cancer DSL to professional associations.
