Job summary

Penn Medicine is seeking a Nurse Navigator in Corporate Services, Cancer Patient Family Services. The role focuses on advocating, educating and supporting oncology patients and families while coordinating care across disease sites.

The Nurse Navigator will apply clinical expertise to patient care, facilitate access to care, and work with providers and staff to ensure timely information and smooth referrals within the Abramson Cancer Center and related services.

Qualifications

• Bachelor's degree required.
• 3+ years of Oncology experience; preferably in an academic medical center.
• Master's degree in Nursing or related advanced degree preferred.

Responsibilities

• Partners with patients, families, the interdisciplinary team and external sources to provide access to care.
• Participates in tumor conferences, patient-centered conferences, and disease-specific meetings.
• Assesses, plans, implements and evaluates nursing care for oncology patients to remove barriers to care.
• Coordinates patient tests, procedures, appointments and treatments across health care disciplines.
• Documents assessments, interventions and outcomes in PennChart and other data systems.
• Collaborates with health care team members to support patient and family needs.
• Facilitates referrals for patient access and coordination within the cancer center.
