Job summary

Mercy is seeking a Registered Nurse (RN) Nurse Navigator for the Oncology department in St. Louis, MO.

This full-time role coordinates care across the cancer journey, educates patients and families, and collaborates with oncologists, infusion services, imaging, and other teams to ensure timely, high-quality care and an exceptional patient experience. You will maintain thorough documentation in EPIC and support program development while addressing barriers to care and providing compassionate

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2-3 years nursing experience.
• BLS certification.

Responsibilities

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, survivorship.
• Educate patients and families regarding diagnoses, treatments, medications, side effects, and support resources.
• Facilitate communication between patients, providers, infusion, imaging, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop care plans.
• Assist with referrals, appointment coordination, and transitions of care.
• Monitor patient progress and address barriers to care.
• Provide emotional support and connect patients with community resources.
• Maintain accurate documentation in EPIC and clinical systems.
• Track patient outcomes and quality metrics to support program development.
