Job summary

Mercy South Sindelar Cancer Center is seeking a compassionate Registered Nurse (RN) Nurse Navigator to serve as a trusted guide for oncology patients and families. You will coordinate care, educate patients, remove barriers to treatment, and support throughout cancer journeys, working with oncology teams to deliver timely, high-quality care.

The role requires RN licensure, 2–3 years of nursing experience, BLS, and often oncology experience; BSN and certifications are preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate and navigate care for oncology patients across diagnosis, treatment, survivorship.
• Educate patients and families on diagnoses, treatment plans, medications, procedures, and side effects.
• Facilitate communication between patients, providers, infusion, imaging, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor patient progress and address barriers to care.
• Provide emotional support and connect to community resources and financial assistance.
• Maintain documentation in EPIC and other clinical systems.
• Track patient outcomes and quality metrics.
• Serve as a clinical resource for patients, families, providers, and staff.
