American Oncology Network, Inc. seeks an LVN/LPN Nurse Navigator in Houston, TX to coordinate oncology care and educate patients. Under physician and RN oversight, you will advocate for timely access to services and provide psychosocial support across the cancer care continuum.

You will collaborate with physicians, nursing staff, patient advocates, and community resources, documenting care in EMR and ensuring smooth transitions across care settings.

Qualificações

• Graduate of an accredited Vocational Nursing program.
• High School Diploma or equivalent required.
• Two years of clinical nursing experience required.
• Current Texas LVN license and BLS certification.
• Oncology/navigation certification preferred, if applicable.

Responsabilidades

• Coordinate care across providers, hospitals, infusion services, home health, hospice and community resources.
• Educate patients and caregivers on diagnoses, treatments and resources.
• Advocate for patient needs and facilitate access to services.
• Document patient interactions and care coordination in the EMR.
• Support transitions of care during admissions, discharges and follow-ups.
