Prisma Health in Richland, SC is seeking an experienced registered nurse to coordinate oncology patient care across the continuum. You will evaluate referrals, participate in multidisciplinary teams, and develop care plans with oncologists and support staff to overcome barriers and improve outcomes.

The role emphasizes patient education, survivorship planning, and engagement with community resources. Requires a Bachelor's in Nursing, an active RN license, and at least three years of oncology

Qualifications

• Bachelor’s degree in Nursing.
• Three (3) years Oncology experience.
• RN license recognized by the NCSBN Compact or licensed in SC.
• Oncology certifications within 1 year of hire: OCN, ONN-CG, TCTCN, or CBCN.

Responsabilités

• Coordinate the care for oncology patients, their family and/or caregiver across the continuum of care.
• Evaluate new patient cancer referrals and review records for NCCN guidelines.
• Assess health status of patients targeted for cancer care management and conduct initial assessments.
• Refer patients to appropriate support services after assessing needs and barriers.
• Participate in multidisciplinary care plans and documentation of treatment plans.
• Lead patient education and collaborate with patients to enhance self-management.
• Promote education for patients, families, staff, and community on oncology care.
• Participate in inter-disciplinary teams to develop new care processes.
• Prepare and lead cancer site-specific tumor boards and participate in teams as needed.
