Qualifications

• RN license in the state of work or multi-state license recognized by NCSBN.
• Must obtain one oncology certification within 1 year of hire (OCN, ONN-CG, TCTCN, CBCN).

Responsibilities

• Coordinate care for oncology patients and families across the continuum.
• Assess patient needs and barriers, initiate referrals to support services.
• Develop and document care plans and survivorship care plans with oncologist input.
• Educate patients and families to enhance self-management and adherence to plan of care.
• Participate in multidisciplinary rounds and tumor boards; contribute to care pathways.
