Job summary

Rush University Medical Center is seeking an Oncology Nurse Navigator 2 in Chicago. This RN role coordinates care across physician practices and multidisciplinary teams to optimize service utilization and patient outcomes within the Cancer Center.

The Navigator supports psychosocial, financial and clinical needs throughout the care continuum, acting as liaison among patients, providers, hospital and community while upholding Rush values.

Qualifications

• Current license to practice as a nurse.
• At least one year of nursing experience required.
• Nursing degree at the baccalaureate level or higher for hires after Jan 1, 2014.
• CPR certification required; some departments may require additional certifications.

Responsibilities

• Establishes a caring therapeutic relationship with patients and families and manages a defined patient caseload.
• Assess patients for social determinants of health to plan care and goals.
• Serve as a liaison among patients, practices, hospital, and community throughout the care trajectory.
• Advocate for timely and equal access to treatment and care.
• Collaborate with the health care team to coordinate patient care and refer to clinical trials as needed.
• Provide psycho-social support and facilitate referrals and transitions of care.
