Job summary

Rush University Medical Center is seeking an Oncology Nurse Navigator II to coordinate patient care across medical oncology, hematology, stem cell transplant, gyne-oncology, surgery, radiation oncology, and cancer screening clinics. The RN will assess psychosocial and clinical needs, liaise with multiple teams, and support patients through the continuum of care.

The role requires a current nursing license, BSN or higher, at least one year of nursing experience, and CPR certification.

Qualifications

• Current license and credentials by the state they are assigned.
• At least one year of nursing experience required.
• Earned Nursing Degree at the Baccalaureate level or higher for those hired after January 1, 2014.
• Transfers to Rush University Medical Center or Rush University Medical Group must have a BSN or higher prior to transfer.
• CPR certification is required.

Responsibilities

• Establishes a caring therapeutic relationship with patients and families.
• Assess patients for social determinants of health and develop a care plan.
• Serve as liaison for the patient throughout diagnosis to end of life or survivorship.
• Advocate for timely access to treatment and equal health care access.
• Collaborate with the health care team to coordinate patient care.
• Coordinate with the clinical trial office and refer as needed.
• Provide psycho-social support and facilitate transitions of care.
