Job summary

Rush University Medical Center in Chicago, Illinois is seeking a Care Management Navigator I to join the Value Based Program. This role supports care coordination, discharge planning, and utilization review across the division, collaborating with physicians, nurses, social workers, payers and patients.

The navigator will contribute to transitional care planning and outreach efforts for a patient list, sharing knowledge to empower patients to navigate services in Rush’s care management model.

Qualifications

• High School diploma or GED is required.
• Knowledge of ambulatory setting is needed.
• Basic knowledge of Microsoft Office required.
• Written, verbal and exceptional interpersonal communication skills.
• Ability to multi-task effectively.
• Ability to function independently, organize workload and stay self-motivated.

Responsibilities

• Daily monitoring of discharges, ER discharges, and referrals.
• Provide outreach for follow-ups and new patient appointments as per policy.
• Prepare and deliver routine medical records to referring agencies and PCPs.
• Document correspondence in electronic medical records as needed.
• Liaison to multiple departments, agencies and provider offices.
• Participate in meetings as assigned.
• Identify at-risk individuals and conduct health risk assessments.
• Assist with insurance processes and connecting patients to resources.
• Coordinate patient transportation as needed.
• Collaborate with CM team to prioritize tasks and meet department needs.
• Flex schedule to meet department requirements.
• Assist with Medicare Message processes and federal mandates compliance.
• Coordinate post-discharge care transition referral communication.
• Triages issues and escalates as appropriate.
• Support payer approvals, denials, and level of care management with secure PHI handling.
