Trinity Health in Florida is seeking a Social Worker (MSW) to join the Oncology Service Line on-site at Holy Cross Medical Group. The role focuses on psychosocial support, resource coordination, and family advocacy to improve patient and family coping during illness.

The ideal candidate holds an MSW with 2+ years of clinical experience in acute care or community settings and possesses strong leadership, communication, and teaching skills.

Qualifications

• MSW from an accredited program; LCSW preferred.
• Minimum of two years of clinical experience in an acute care hospital or community setting.
• Hospital experience preferred.
• Proficient computer skills.
• Demonstrated leadership, communication, and teaching abilities.

Responsabilités

• Responds to referrals by assessing and providing resources to patients at high risk for complex psychosocial needs in collaboration with the RN Case Manager.
• Provides counseling, emotional support, and education regarding adjustment to illness.
• Prepares the family for the reality of caring for the patient on an on-going basis.
• Advocates for the patient's rights and entitlement.
• Participates in team/family conferences.
