Job summary

Holy Cross seeks a full-time Social Worker to support oncology patients and families at the Holy Cross Medical Group Oncology office in Fort Lauderdale. The role focuses on psychosocial assessment, resource coordination, counseling, and advocacy to assist patients with illness adjustment and family preparedness.

Requirements include an MSW, at least two years of clinical experience in acute care or community settings, and a background in case management and hospital work.

Qualifications

• MSW from an accredited school is required.
• Minimum of two years clinical experience in an acute care hospital or community service setting.
• Must have MSW, previous case management experience and hospital experience preferred.
• Must have computer skills.
• Leadership, communication, and teaching abilities are necessary.

Responsibilities

• Responds to referrals by assessing and providing resources to patients at high risk for complex psychosocial needs, coordinating with RN Case Manager.
• Provides counseling, emotional support, and education regarding adjustment to illness.
• Prepares the family for ongoing patient care.
• Advocates for patient rights and entitlement.
• Participates in team/family conferences.
