Job summary

Holy Cross Health in Fort Lauderdale, FL is seeking a Social Worker with a Master of Social Work (MSW) and Florida LCSW licensure for a full-time Day Shift position in our Oncology Service Line.

Qualifications

• MSW degree required and Florida LCSW licensure preferred.
• Minimum of two years clinical experience in acute care or similar setting.
• Hospital and case management experience preferred; strong communication skills.
• Must have computer skills and be able to use electronic records.
• Demonstrated leadership and teaching abilities.

Responsibilities

• Responds to referrals by assessing and providing resources to patients with complex psychosocial needs.
• Identifies financial and community resources with RN Case Manager collaboration.
• Coordinates resources not available via usual services and liaises with patient/family and agencies.
• Provides counseling, emotional support, and education on illness adjustment.
• Prepares families for ongoing patient care and advocates for patient rights and entitlements.
• Participates in team and family conferences.
