Qualifications

• PharmD degree is required from an accredited school of pharmacy.
• California Pharmacy License and active ASHP/CSHP membership required.
• Two years hospital pharmacist experience required.
• Residency or oncology board certification preferred; publication in peer-reviewed journals is a plus.

Responsibilities

• Demonstrates compliance with Code of Conduct and policies and reports violations.
• Ensures cost-effective purchasing.
• Maintains appropriate inventory levels including just-in-time inventory.
• Ensures prescribing is accurate and appropriate for patient age and condition.
• Checks for allergies, interactions and duplications in prescriptions.
• Oversees pharmaceutical preparation and dispensing per policy.
• Administers high-cost oncology and infectious disease medications safely.
• Enforces 797 compliance and standards.
• Collaborates with multidisciplinary team for safe, cost-effective med use.
• Documents patient-specific interventions with the team.
• Provides patient education and timely medication information.
• Manages shift tasks and priorities efficiently.
• Protects patient confidentiality and privacy.
• Performs other duties as assigned.
