Job summary

Fred Hutchinson Cancer Center is seeking a Pharmacist for a .5 FTE role based in Issaquah, WA. The pharmacist will work with the specialty pharmacy team to provide clinical support, drug therapy management, and education to providers, staff, and patients.

The position emphasizes patient-centered pharmacotherapy, medication safety, and collaboration within a multidisciplinary care team. This role requires a WA pharmacist license and a PharmD or equivalent.

Qualifications

• Graduation from an accredited School of Pharmacy is required.
• State of Washington Pharmacist License is required.
• Two years of experience in pharmacy practice (months spent in an ASHP-accredited residency can substitute month-for-month).

Responsibilities

• Collaborate with providers to individualize drug therapies for safe, effective patient care.
• Perform comprehensive medication therapy review and ensure safe transitions of care.
• Monitor and modify drug regimens, order and interpret laboratory tests as needed.
• Provide refill authorization after assessing adherence and safety.
• Serve as drug information resource to providers, nursing staff, and patients.
• Ensure access to specialty medications by providing clinical information to insurers.

Education

Graduation from an accredited School of Pharmacy

Doctor of Pharmacy (PharmD) degree or equivalent
