The role requires RN licensure, CCM certification within 36 months of hire, 5+ years of relevant experience, CBT/MI/DBT training, and proficiency with EHR/case management software. Office-based or remote role with HIPAA compliance.

Qualifications

• 5+ years in clinical, case/utilization, or disease management or related health ops experience.
• Experience working with diverse populations.
• Advanced training in CBT, motivational interviewing, or DBT.
• Clinical assessment and care planning expertise.
• Proficiency with Microsoft Office, case management software and EHRs.
• RN licensure or higher with CCM certification timeline.
• Strong understanding of healthcare systems and case management.

Responsibilities

• Manage a caseload of members with complex health needs and coordinate care.
• Assess health status and develop individualized care plans in collaboration with providers.
• Implement, monitor and adjust care plans to meet goals and access to services.
• Advocate for members and liaise with physicians and the care team.
• Document activities in compliance with policies, HIPAA, and regulatory standards.
• Mentor and provide guidance to junior Case Managers.
• Serve as a clinical resource and problem-solver for the team.
