Job summary

LHC New Liberty Hospital Corporation is seeking a Cancer Care Pharmacist to provide comprehensive pharmaceutical care for patients at its Westwood Administration – East location. The role emphasizes safe, effective, and cost-conscious pharmacotherapy with strong inter-disciplinary communication.

The pharmacist will participate in rounds, perform medication reviews, and support educational initiatives and residency rotations, while adhering to safety and quality processes across the department.

Qualifications

• Bachelor’s Degree in Pharmacy is required.

Responsibilities

• Actively participates in performance improvement efforts to strengthen pharmaceutical care and policies.
• Participates in multidisciplinary rounds and provides patient-focused pharmaceutical care.
• Supports staffing needs by flexing to other areas or shifts as needed.
• Ensures timely medication order processing through order entry and verification.
• Performs patient-specific medication reviews including dosing adjustments for renal/hepatic dysfunction.
