Job summary

The Ohio State University Wexner Medical Center is seeking a Care Management RN in the Oncology program (James). The role coordinates patient care from pre-admission through post-discharge, ensuring timely decisions and efficient resource use.

The Care Manager collaborates with physicians and nurses, communicates the plan of care to third-party payers, and supports appropriate levels of care using evidence-based criteria.

Qualifications

• RN licensure required and two years of registered nursing experience
• Five years of recent clinical experience preferred in relevant population
• Knowledge of clinical practice guidelines and payer protocols desired
• Proficiency with computer databases and health information systems

Responsibilities

• Coordinate care from pre-admission through post-discharge
• Collaborate with physicians, nurses and other staff to manage patient transitions
• Identify and remove barriers to the plan of care for each patient
• Serve as central information source for third-party payers and obtain approvals
• Assist physicians in ensuring appropriate level of care using evidence-based criteria
