Job summary

The Ohio State University Wexner Medical Center is seeking a Care Management RN - Oncology to coordinate patient care across the continuum in an outpatient setting at the James Cancer Hospital.

Responsibilities include monitoring patient progress, collaborating with physicians and care teams, and applying evidence-based practices to ensure safe, high-quality care while supporting efficient resource use.

Qualifications

• Bachelor's degree in Nursing; RN licensure required; two years RN experience.
• Knowledge of evidence-based practice and disease-specific processes.
• Technology support and payer protocols desired.

Responsibilities

• Coordinate care across the continuum from pre-admission through post-discharge for assigned patients.
• Monitor patient progress and care delivery, documenting and communicating key information.
• Collaborate with physicians, nurses, and interdisciplinary teams for assessments and planning.
• Stay aware of healthcare trends and drive initiatives to improve care quality and resource utilization.

Education

Bachelor's degree in Nursing

RN licensure

Masters preferred
