Job summary

Mount Sinai Health System is seeking a Clinical Coordinator RN to coordinate complex outpatient oncology care pathways at Mount Sinai Hospital in New York.

You will collaborate with Nursing, Social Work, and Care Management to set goals, develop policies, and lead day-to-day care coordination. A Bachelor’s in Nursing is required, with 2 years in hematology/oncology or BMT; NYS license and BCLS certification.

Qualifications

• Bachelor's degree in Nursing required
• Master's degree in health-related field preferred
• 2 years experience in hematology oncology or BMT
• Current NYS nurse licensure
• BCLS certification required (AHA)

Responsibilities

• Coordinate clinical care within a designated specialty with Nursing, Social Work, and Care Management.
• Establish and implement short and long-term goals, objectives, policies, and procedures.
• Organize and lead day-to-day patient care coordination.
• Provide consultation and education to the interdisciplinary team regarding specialty patient populations.
• Promote an interdisciplinary approach to patient care delivery.
• Serve as a resource for patients and their families.
• Act as a patient advocate utilizing the Patient Bill of Rights.
• Participate in promoting and maintaining a safe and therapeutic environment.
• Collaborate in the development and implementation of nursing standards.
• Maintain leadership competency through continuing education and professional organizations.
