Job summary

Qualifications

Responsibilities

• Coordinates clinical care within a designated specialty in collaboration with Nursing, Social Work, Care Management, and related disciplines.
• Develops and implements short- and long-term goals, policies and procedures to ensure high nursing care delivery.
• Leads day-to-day patient care coordination aligned with program objectives and clinical expertise.
• Provides education to team members on the clinical specialty service and care coordination.
• Promotes interdisciplinary care delivery and acts as a patient advocate.
• Engages in professional development and keeps abreast of current practice trends.
• Supports policy development, quality improvement, and readiness for surveys.
