Job summary

Southern Illinois Healthcare is seeking a licensed counselor to assess, counsel, and provide therapy to individuals, families, and groups. The counselor collaborates with primary care providers, psychiatric staff and other multidisciplinary team members to support whole-person care and improve patient outcomes.

The position involves comprehensive biopsychosocial assessments, crisis intervention, and care coordination, with a focus on evidence-based practices and community referrals.

Qualifications

• Master’s Degree in Social Work or Counseling required with IL licensure (LCSW/LCPC).
• Minimum two years of individual, family, or group therapy experience.
• Strong interpersonal communication and collaboration with medical teams.
• Knowledge of trauma-informed and recovery-oriented care.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and treatment planning.
• Provide individual, family, and/or group therapy for behavioral and mental health needs.
• Coordinate with medical providers to ensure continuity of care.
• Provide crisis intervention, risk assessments, and safety planning as needed.
• Refer patients to higher levels of care or community resources when appropriate.
• Maintain HIPAA compliance and ethical/legal standards.
• Utilize evidence-based practices to evaluate care and guide treatment.
• Develop partnerships with community organizations for continued care.
• Collaborate on discharge planning with patients and post-discharge staff.
