Lee Health is seeking an experienced Ambulatory RN Care Coordinator to join our oncology team and serve as a key resource for patients throughout their cancer care journey in Bonita Health Center, Estero. This role emphasizes coordinating care across the continuum, providing telephone triage, advocating for patients and families, and managing clinical messages to support timely, high-quality, patient-centered care.

Qualifications

• Minimum of 3 years in clinical setting with emphasis on patient education and chronic disease management.
• Active Florida RN license required.
• Care coordination experience preferred.
• Strong communication and triage abilities.

Responsabilités

• Coordinate care for oncology patients across the continuum of care.
• Serve as a clinical resource and advocate for patients and families.
• Provide telephone triage and respond to patient questions, concerns, and clinical needs.
• Manage and respond to in-basket messages and coordinate appropriate follow-up.
