Lee Health in Estero, FL is seeking an experienced Ambulatory RN Care Coordinator to join the oncology team. You will coordinate oncology patient care, serve as a resource for patients and families, and provide telephone triage and follow-up coordination in a collaborative office setting.

The role requires at least 3 years of clinical nursing experience, Florida RN licensure, and a focus on patient education and care coordination within a high-risk patient load.

Qualifications

• Minimum of 3 years in a clinical setting with emphasis on patient education and chronic disease management.
• Experience in oncology or care coordination is preferred.
• Strong communication and advocacy for patients and families.
• Ability to manage multiple tasks and coordinate across care teams.

Responsabilités
