Lee Health is seeking an Ambulatory RN Care Coordinator at the Bonita Community Health Center in Estero, FL. This full-time role supports oncology patients through complex care journeys and acts as a clinical resource and advocate within the office setting.

The position requires at least 3 years of RN experience, oncology experience preferred, and case management/nurse navigator experience preferred. Relocation and sign-on bonuses are available; eligible for excellent benefits.

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.
