Job summary

Nurses at Home is seeking a full-time RN Clinical Case Manager to deliver high-quality home health care on the road across Connecticut. If you can complete OASIS proficiently and bring a positive attitude, you’ll join a small, clinician-first agency with direct owner contact and top-of-market pay.

Qualifications include RN license, English proficiency, Bachelor’s degree preferred, and at least 2 years in RN nursing preferred.

• RN license required.
• OASIS proficiency preferred.

Responsibilities

• Provide comprehensive home health nursing care as an RN clinical case manager.
• Coordinate and monitor patient care on the road across CT.
• Complete OASIS assessments proficiently.
• Participate in weekend on-call rotation as scheduled.
