Tudor Oaks Senior Living Community in Wisconsin is seeking an MDS Coordinator (RN) to oversee the interdisciplinary MDS assessment and care planning processes on-site.

This full-time role focuses on timely, accurate MDS transmission, compliance with federal and Medicare regulations, and collaborative care to support resident outcomes. Schedule is Monday–Friday, 8:00 a.m.–4:30 p.m., on-site, with a competitive salary and benefits.

Qualifications

• Active, unencumbered RN license (Wisconsin or transferable).
• At least 1 year of MDS nursing experience.
• Strong clinical judgment and attention to detail.
• Able to prioritize and manage multiple deadlines.
• Effective communication and teamwork skills.

Responsibilities

• Coordinate and oversee the interdisciplinary MDS assessment and care planning process.
• Ensure timely and accurate completion and transmission of MDS assessments.
• Maintain compliance with federal, state, and Medicare regulations.
• Collaborate with nursing and interdisciplinary teams to support quality outcomes.
• Monitor assessment timelines and address changes in resident condition.
