Job summary

UnitedHealth Group in Minnetonka is seeking an experienced RN Case Manager to serve as the on-site clinical presence and coordinate high-quality patient care within the primary care team.

The role includes nurse-led visits, preventive care, and chronic disease management, with a schedule Monday–Friday, 6:45 a.m. to 3:15 p.m. CST. Minnesota licensure, 2+ years of related experience, and EMR proficiency are required.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unrestricted RN license in the state of employment.
• 2+ years of nursing experience in primary care or related settings.
• Experience using electronic medical records.
• Strong communication and customer service skills.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Serve as on-site RN and primary clinical presence at Minnetonka Well clinic.
• Conduct nursing visits including biometric screenings, vaccinations, health coaching, and triage.
• Coordinate care among patients, providers, specialists, pharmacies, and resources.
• Support prior authorizations and follow up on orders and test results.
• Provide education on preventive care and chronic disease management.
• Maintain accurate documentation in the EMR and related systems.
• Assist with onboarding and mentoring staff as needed.
• Ensure compliance with HIPAA, OSHA, and regulatory requirements.
