Job summary

MercyOne Northeast Iowa in Waterloo, IA is seeking a part-time nurse to join a 24/7 nurse advice and triage team. The role provides telephone guidance from an on-site location, assesses symptoms, and helps patients navigate next steps, including scheduling with new providers when appropriate.

You'll work 7 pm–7 am, 0.6 FTE, with every third weekend rotation and two holidays per year. Requires ADN, Bachelor’s preferred, and a minimum of three years of nursing experience, plus independence and

Qualifications

• Associate's degree in Nursing required.
• Bachelor's degree preferred.
• Minimum of 3 years of nursing experience.
• Ability to work independently with confidence in nursing skills.

Responsibilities

• Receives, reviews and acts on discharge reports needing follow-up.
• Reviews reports for ACO and PCMH programs.
• Conducts telephone triage and initial data gathering.
• Develops outcome-based care plans from identified goals.
• Educates patients and families to meet needs.
• Performs pre-visit chart reviews for targeted patients.
• Collaborates with Clinic Medical Home Team and adheres to safety policies.
• Provides age-appropriate care per patient needs.
