Rcm-of-Washington-In seeks a Job Coach to provide person-centered supports for individuals with disabilities, helping them retain employment through on-site coaching and assessments.

Reports to the RCM Director or Community Inclusion Manager. Requires 1 year of disability-focused experience, strong English communication, and MS Office proficiency. Scheduling aligns with client needs and program hours. Training and documentation are essential parts of the role.

Formación

• High school diploma or GED required.
• One year of work experience with disabilities and case management knowledge preferred.
• Proficient with MS Word, Excel, Access, and Outlook.

Responsabilidades

• Provide person-centered supports to help individuals with disabilities retain employment.
• Perform on-the-job coaching and direct support at the work site as needed.
• Document daily notes, assessments, reports, and billing; maintain electronic records.
• Monitor performance against goals and employer requirements; provide regular feedback.
• Coordinate with service coordinators and support teams; participate in trainings and meetings.
