Job summary

Evolve Esolutions LLC in Fremont, California is seeking a Certified Occupational Therapy Assistant (COTA) to join our 126-bed facility. You will work under the supervision of an Occupational Therapist to implement treatment plans and help patients regain independence through therapeutic activities and daily living skills.

This on-site, full-time role requires an associate degree, state licensure, and at least two years of healthcare experience; SNF experience preferred.

Qualifications

• Associate's degree from an Occupational Therapy Assistant program.
• Current state license or certification as an Occupational Therapy Assistant.
• Minimum of 2 years of experience in a healthcare setting; SNF experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of occupational therapy principles, practices, and treatment techniques.
• Ability to perform physically demanding tasks, including lifting, transferring, positioning, and assisting patients.
• Familiarity with EMR systems and clinical documentation.

Responsibilities

• Assist Occupational Therapists in implementing individualized treatment plans for patients.
• Conduct therapy sessions and therapeutic activities under the direction and supervision of the Occupational Therapist.
• Monitor patient progress and report any changes in condition or functional abilities to the supervising Occupational Therapist.
• Educate patients and their families on therapeutic activities, exercises, and techniques to improve function and independence.
• Maintain accurate, complete, and timely documentation of patient progress and treatment plans.
• Collaborate with other healthcare professionals to ensure comprehensive, coordinated, and effective patient care.
• Maintain a clean, safe, and organized therapy area.
• Stay informed about current trends, best practices, and advancements in occupational therapy.
• Follow established facility policies, procedures, and applicable professional standards.
