Job summary

Oakwood Gardens Care Center is seeking a caring Certified Occupational Therapy Assistant (COTA) to join our skilled nursing rehab team in Fresno, CA. You will treat patients under OT guidance, document progress daily and weekly, and implement treatment plans while coordinating with nurses and therapists.

You will also train restive aides, communicate with the health team, and participate in discharge planning to ensure smooth transitions for residents.

Responsibilities

• Treat patients as directed by the Occupational Therapist.
• Record daily treatment notes and weekly progress notes per OT Board.
• Treat patients per physician treatment plan.
• Assist nursing with Restorative Aide training.
• Communicate with supervisor and health team about patient progress and plans.
• Participate in staff in-services and training.
• Record evaluations, daily notes, weekly progress, and recertification docs per policies.
• Instruct families or nursing staff in maintenance and caregiver training for discharge.
• Participate in discharge planning.
• Assist with cleaning and maintenance of treatment area.
• Report equipment problems to keep department functioning.
• Relate positively with patients, families, staff and colleagues.
