Job summary

Volunteers of America Los Angeles is seeking a Case Manager (Specialist) to provide intensive on-site services to eligible clients, meeting weekly and coordinating care across resources.

You will conduct screenings, develop action plans, link services, advocate with agencies, and document progress while supporting a vulnerable population in LA. Fingerprint clearance and related education required.

Qualifications

• Must be able to pass a fingerprint clearance and background check.
• Ability to manage emotional stress when dealing with challenging clients.

Responsibilities

• Conduct comprehensive screenings and assessments to develop a case plan.
• Provide service linkages and support systems to meet identified needs.
• Coordinate planning and case conferences with residents and the service team.
• Advocate for clients before agencies and community bodies.
• Coordinate care, document sessions, and motivate client progress.
• Engage participants in activities and group education sessions.
• Carry out other duties as required.

Education

Associate’s Degree in Social Work or related field

Two years’ total education and relevant experience
