Job summary

Qualifications

• Three (3) years of clinical experience with medical terminology, diagnoses, treatments, and disease processes.
• Master of Social Work or LMSW/LCSW licensure (Missouri) as one acceptable credential.
• Nursing or therapy licensure options listed in the posting.
• Additional licenses/certifications may be required by the department.

Responsibilities

• Assess and screen admissions to support early transition planning and reduce readmission risk.
• Develop and implement care plans with patients, families, and care teams.
• Coordinate services and ensure timely provision of care and resources.
• Facilitate discharge planning and transitions for complex cases.
• Collaborate with physicians, home care agencies, and other providers to meet patient needs.
• Communicate with payers and document compliance and utilization.
