C Cassia Community is seeking a Speech Language Pathologist to float between Truman Senior Living and Park View Care Center in Truman, MN. The role offers on-call hours with a wage range of $45–$55 per hour, based on experience, plus mileage reimbursement.

The position requires CCC-SLP, MN license, and a relevant degree. Responsibilities include assessment, treatment planning, therapy provision, and collaboration with healthcare teams.

Qualifications

• Must be a graduate from an Accredited Speech Therapy Program and have a bachelor’s degree.
• Must possess a current accredited MN Speech Therapist License.
• Homecare Homebase EMR experience helpful but will train.
• Experience with OASIS preferred or willing to be trained.
• Must possess strong knowledge regarding state, federal, and local regulations as they pertain.
• Develop care plans and discharge planning strategies for optimal patient recovery.
• Collaborate with healthcare professionals to ensure comprehensive and personalized treatment plans.
• Demonstrate excellent communication skills to educate patients and their families on therapy techniques.

Responsibilities

• Assess & Diagnose Communication Disorders: Evaluate patient needs and deficits.
• Develop & Implement Treatment Plans: Create personalized therapy programs.
• Provide Speech & Language Therapy: Conduct exercises and interventions.
• Educate & Support Patients & Families: Teach strategies for effective communication.
• Monitor Progress & Adjust Treatment: Track progress and modify plans.
• Collaborate with Healthcare Teams: Work with doctors, nurses, and therapists.
