Job summary

Cassia Community is seeking a Speech Language Pathologist to float between Cokato and Dassel. Competitive wage ($36-$46/hr) with benefits, mileage reimbursement, and a $2,500 bonus paid over 18 months. Full-time or part-time, benefits eligible.

Must be a graduate of an Accredited Speech Therapy Program with a Bach­elor’s degree and hold a current MN SLP license. Experience with Home Health care and EMR is helpful.

Qualifications

• Must graduate from an accredited Speech Therapy program with a bachelor's degree.
• Current MN Speech Therapist License required.
• Homecare Homebase EMR experience is helpful but not required.
• Experience with OASIS preferred or willing to be trained.
• Knowledge of state, federal, and local Home Health regulations.
• Ability to develop care plans and discharge planning.
• Collaborate with healthcare professionals to ensure comprehensive treatment.
• Excellent communication skills (verbal and written) in English.
• Capable of performing essential functions with or without accommodations.

Responsibilities

• Assess and diagnose communication disorders and swallowing difficulties.
• Develop and implement personalized therapy treatment plans.
• Provide direct speech and language therapy to patients.
• Educate patients and families on strategies for communication and home care.
• Monitor patient progress and adjust treatment as needed.
• Collaborate with doctors, nurses and therapists to coordinate care.
